Study of Instructional Improvement

The Student Motivation Form (SMF) was designed to provide information based on self-reports of how students perceive themselves in terms of academic interests or skills. The SMF form was administered each spring as part of the assessment. Teachers were asked to complete a Student Rating Form (SRF) for each student for whom they filled out an instructional log. The SRF instrument gathers information on a student’s academic engagement, approaches to learning, problem behaviors (if any), and compensatory services received.
